After serving in the Israeli army, he developed a successful real estate business, while at the same time, beginning in 1980, he starts producing films and studies at the Camera Obscura School of Tel Aviv. Among his best known film sare: Nagua, Bar 51 and Himo the King of Jerusalem (by the filmmaker Amos Gutman), the film The Elected (with Daniel Waksman). He was the director and screenwriter of the film Revenge of Itzik Finkelstein, which won 7 awards from the Israeli Film Academy (including Best Film, Best Director and Best Screenplay) and represented Israel in 1994 for the Oscar.
In 1993, he arrived to Cuba where he currently lives. In Cuba, he built the Miramar Trade Center, a major business complex in the city of Havana, which he still runs to this day. In this center he also creates a significant Cuban art collection for the country.
He writes the novel Cejalinda, published under the title: La mujer de su vida, Quarto, Spain, in 2006.
In Cuba he began his photographic work. In 2010, his series Sleeping with… (Dormir con) is exhibited at the Cuban Photo Library (National Photography Museum), which now is part of the collection of this institution, and of numerous private and institutional collections along with other works of his. His first series Sleeping with... was exhibited in 2010 at the Photo Gallery of Cuba becoming part of the collection of this institution, and was chosen by Discoveries of PhotoEspaña 2011. Today his work has wide recognition amongst the public and critics. He took part in the past Havana Biennial 2012, with two of his series, The Family and Self-portraits.
Among the most important series are: the Self-portraits (2011–2014), The Family (2011–2013), Forgotten (2013), Cuts (2013–2014) as well as large format works such as The Line (2014), Centipede (2014) and photo installations: 19 women and one bed (2012) and The dance (2014).
He collaborates with the new cultural project “Cuban Art Factory” (Fábrica de Arte Cubano), promoting the development of contemporary photography and visual arts in Cuba.
She is the daughter of Cuban president Raúl Castro and feminist and revolutionary Vilma Espín as well as the niece of former president Fidel Castro. She has a brother, Alejandro Castro Espín.
Her group campaigns for effective AIDS prevention as well as recognition and acceptance of LGBT human rights. In 2005, she proposed a project to allow transgender people to receive sex reassignment surgery and change their legal gender. The measure became law in June 2008 which allows sex change surgery for Cubans without charge.
Mariela Castro is president of the Cuban Multidisciplinary Centre for the Study of Sexuality, president of the National Commission for Treatment of Disturbances of Gender Identity, member of the Direct Action Group for Preventing, Confronting, and Combatting AIDS, and an executive member of the World Association for Sexual Health (WAS). She is also the director of the journal Sexología y Sociedad, a magazine of Sexology edited by her own National Center for Sex Education (CENESEX).
She has published 13 scholarly articles and nine books.
She is also a sitting member of the National Assembly of People's Power. When the assembly voted in 2014 to ban discrimination on the basis of sexual orientation in employment, Castro opposed the legislation because it did not also include protection on the basis of gender identity, and became possibly the first legislator in the body's history ever to vote against a piece of legislation.
Mariela is now married to Italian Paolo Titolo, General Manager of Amorim Negócios Internacionais, S.A. in Cuba, and has two children with him, and a daughter from her previous marriage with the Chilean former FPMR member Juan Gutiérrez Fischmann.
In 2010 the Cuban government began providing sex reassignment surgery free of charge as part of their universal healthcare. This was the result of several years of work by the Cuban National Center for Sex Education under the leadership of Mariela Castro. The current developments in LGBT rights in Cuba are remarkable given the discrimination suffered by gays, lesbians, and transgender people in Cuba in the 20th century, as well as comparison with current LGBT movements in the U.S. and abroad.
